Physiotherapist - Stroke Outpatient Clinic
POSITION SUMMARY :
Physiotherapist : Mackenzie Health Outpatient Stroke Program
We are currently seeking a Physiotherapist to work at Mackenzie Health Hospital in Stroke Outpatient Clinic located in Vaughan.
The Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients.
The successful candidate will be familiar working in an outpatient clinic setting.
As the successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario.
You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.
RESPONSIBILITIES :
- As a Physiotherapist (PT) you will be conducting assessments & treatment plans
- As a PT you will be formulating goals and recommendations
- Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants
- Assist in the promotion of physiotherapy services to external referral sources
- Participating actively in the planning, implementation and evaluation of the program
REQUIREMENTS :
- Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team
- Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario
- Minimum 2 years clinical experience in Stroke / Neurological patient populations is required
- Experience in assessment and generating treatment plans for patients with stroke / neurological disorders
- Knowledge of stroke best practices
- Experience with Chronic disease management is a good asset
- Sound knowledge of community resources is an asset
- NDT training is an asset
- Effective interpersonal, organizational and communication skills
- Strong customer service focus and excellent problem-solving skills required
- A positive work and attendance record is essential
- Ability to adapt to new computer applications
- CPR + AED certificate
- Valid G license required
- Criminal background check, Vulnerable sector check and Immunization record (current).
