Job Summary : The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals.

The PSW will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program.

This support relates to the physical, cognitive, mental health and social / recreational needs of the clients.

The staff member will maintain the established routines of the program including all security, fire and general health and safety practices.

The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.

LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program .

The standard Service Period for a H2H Program is sixteen (16) weeks in duration, with one Service Bundle to be assigned to the Patient and delivered over the course of this defined timeframe, with Services provided in accordance with the Patient Transitional Care Plan.

The H2H program is designed to support adults 55+ facing complex mental health challenges, addiction, dementia and responsive behaviours along with physical health and care needs.

This program will focus on individuals lacking access to appropriate supports while living in the community and other social determinants of health.

The H2H LOFT Service Team will be working closely with CAMH partners as part of an integrated care model and ensures contact with referred individuals in the community within 24 hours. As part of this mobile team, staff will be travelling across the GTA to meet and work in a variety of environments to support individuals where they are at including in their homes.  The Path Home is a 36-unit ALC transitional housing program designed to provide supportive care to individuals living with mental health challenges.

Our program aims to create a nurturing environment where clients can transition from acute care settings to a more stable living arrangement.

Our program operates in partnership with the Centre for Addiction and Mental Health (CAMH) and the University of Health Network (UHN) hospitals, exclusively accepting referrals from these organizations.

Our dedicated nursing staff offers medical monitoring, wound care, and medication management, while clients also have access to primary care services and case management services.

Life skills training and community engagement activities empower our clients to live independently and experience meaningful connections.

Our program aims to holistically address physical, mental, and emotional needs, fostering a smoother transition, collaborative care, and empowerment for a successful reintegration into the community What You Will Do : Assist clients with physical and mental health needs by fostering respectful relationships, supporting daily living activities, managing medical and behavioral issues, facilitating social engagement, and maintaining accurate care documentation in collaboration with the support team.

Maintain safety and security by responding to emergencies, documenting incidents, ensuring client well-being through regular interaction Oversee site-specific medical responsibilities by monitoring the medication program, ensuring proper storage and documentation, and collaborating with physicians, pharmacists, and program staff to resolve any issues.

Coordinate and participate in social and recreational activities by attending planning meetings, maintaining participant records, ensuring equipment readiness, communicating client absences, leading outings, including driving clients Perform various operational and support duties by managing communications, maintaining daily residence operations, coordinating emergency evacuations, participating in meetings and training, working rotating shifts across housing sites, and accepting delegated tasks from senior staff to ensure smooth program delivery.

Other ad-hoc tasks as assigned by the program manager What You Bring : A diploma / certificate or above in Human Services, Gerontology or Personal Support Worker from a recognized college or university is preferred First Aid and CPR training Experience with clients with unique special needs including cognitive impairment and / or mental health issues.

Knowledge of medication, general health and psychiatric conditions Ability to organize activities and work with individuals and groups in the areas of assessment, problem solving and crisis management; Ability to interact with clients in a positive and supportive manner; Demonstrated ability to work with multi-disciplinary professionals and other staff in a team environment;  Good oral and written English skills.

Computer skills are required Possession of a valid Ontario Driver's License preferred; Work Environment : Physically able to perform demanding body mechanics (e.g.

lifting, transferring, etc.) Since the position involves working in client / client / member’s own homes, staff may be subjected to second hand smoke while performing their job duties.

In addition staff will be required to work rotating shifts as scheduled by the program Staff will be required to work in client’s own homes and may therefore be exposed to secondhand smoke.

Occasional evening and weekend hours may be required.
