Position Summary

Reporting to Nursing Leadership, the Registered Practical Nurse will provide excellent nursing care to residents at the Lakeridge Gardens 320 bed long term care home being built on the Ajax‑Pickering hospital site. This new home is part of the government’s plan to improve long‑term care delivery and capacity in Ontario and in the Durham Region. This state‑of‑the‑art long‑term care home sets a new bar for seniors’ care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features. Just as Lakeridge Health is a beacon in the Durham community, the new long‑term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.

Duties and Responsibilities

• Assessing, planning, implementing and evaluating to provide high quality nursing care to residents;
• Assisting in the development and maintenance of an appropriate plan of care for each resident Ensuring resident care plans are initiated in collaboration with other members of the multidisciplinary team;
• Communicating with relatives regarding the care and condition of respective residents as required;
• Participating in discharge planning, consultation, and / or transfer of residents;
• Providing direct resident care and participate in resident teaching as required;
• Encouraging active participation by residents in all therapy programs and recreational activities;
• Providing first aid to staff when necessary and completing Employee Injury Reports;
• Procuring and administering medications including injections and narcotics;
• Participating in the orientation, on‑the‑job training and in‑servicing of nursing staff as required.

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and C.U.P.E Local 6364.

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
