Job description

Reporting to the Regional Director of the Toronto West Stroke Network the Regional Education Coordinator (REC) leads the planning coordination and evaluation of education initiatives that build stroke care knowledge and clinical expertise across the care continuum. The REC drives and supports sustainability of local and regional education and quality improvement efforts aligned with the Canadian Stroke Best Practice Recommendations. Building relationships and collaborating with providers educators and system leaders to identify learning needs and promote knowledge sharing is a key component of this role. The REC also acts as an active leader and participant in regional and provincial committees. Finally the REC advances knowledge translation activities that strengthen stroke care with the ultimate goal of improving patient outcomes.

Duties:

  • Supports and assists with development implementation and evaluation of regional educational initiatives in the Toronto West Regional Stroke Network (TWSN);
  • Facilitating knowledge translation through the ongoing development and dissemination of Stroke Best Practices in the TWSN region and Toronto Stroke Networks; Working collaboratively with the regional teams of the other Toronto Stroke Networks (North & East GTA Stroke Network and South East Toronto) to support strategic priorities;
  • Contribute to the ongoing development and implementation of a Knowledge Translation and Education Strategy across the Toronto Stroke Networks;
  • Promoting the development of joint education initiatives and activities to foster dissemination and integration of stroke best practices and evolving evidence to be developed in conjunction with other stakeholders in the GTA;
  • Collaboratively developing a formal and sustainable Stroke Virtual Community of Practice that supports stroke best practices
  • Participating and contributing to education related stroke initiatives at a provincial level through the Ontario Regional Education Group (OREG); planning and coordinating responsibilities to facilitate the implementation of best practices stroke care within local organizations within the region and across the continuum of care.
  • Complete other initiatives as identified by the Regional Director TWSN
  • Health promotion and primary stroke prevention secondary stroke prevention emergency and acute stroke care stroke rehabilitation and community re-integration
  • Performing coordination and communication activities


Qualifications :

  • Masters degree (preferred) in any health or education related program
  • Baccalaureate degree in health related discipline required
  • Licensed member in good standing with a professional college and/or licensed member under the Regulated Health Professions Act
  • Experience in the health care system
  • Demonstrated expertise in program and project evaluation required.
  • High level of competency in the use of PC software including Microsoft Office applications (Word Excel Outlook Access Powerpoint) and use of the internet. Demonstrated knowledge of data collection use of databases data analysis and interpretation of data preferred
  • Demonstrated coordination and networking skills
  • Demonstrated skills as an educator and project management skills required. Experience in delivering educational presentations with demonstrated presentation skills
  • Demonstrated coaching and mentoring skills an asset
  • Exemplary interpersonal/communication skills both written and oral. Experience facilitating virtual communication an asset
  • Demonstrated expertise in knowledge translation and adult education principles
  • Expertise in stroke care and demonstrated knowledge of the continuum of stroke care
  • Previous experience as a change agent with an understanding of organizational cultures including working with culturally diverse populations
  • Proven flexibility and ability to adapt positively to change
  • A valid drivers license and own transportation an asset
