The salary range for this position is CAD $54.49 - $68.10 / hour.

Job Summary

We have a full‑time opportunity for a Clinical Pharmacist to join our team at Chilliwack General Hospital located in Chilliwack, BC.

Responsibilities

  • Individualize patient medication therapy by reviewing literature, patient charts, laboratory results, and by meeting with patients and the health care team to assess clinical response.
  • Provide patient medication counselling, summarizing information, demonstrating procedures, and assessing patients’ understanding.
  • Consult with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clinical responses.
  • Assist the health care team in developing drug therapy guidelines for individual disease conditions by reviewing current literature and providing recommendations.
  • Participate in ward rounds, program team meetings and provision of pharmacy services at the unit level to enhance patient care.
  • Act as a department resource person in the assigned area, providing detailed drug information on issues related to drug therapy in those disease states treated within the program.
  • Contribute case studies and review articles to professional journals by identifying topics, gathering and analysing data, reviewing current literature, and writing and submitting articles for publication.
  • Provide instruction and/or supervision to pharmacy students and residents by preparing educational material, demonstrating procedures, lecturing, supervising during patient care activities, and providing input into performance evaluations.
  • Participate in quality improvement activities, performing quality assurance audits, identifying potential problems and recommending changes.
  • Perform drug distribution duties, including interpreting medication orders, dispensing medications using a computerized medication information system, and ensuring compliance with legal and departmental policies.
  • Review prescriptions for therapeutic suitability by referring to standardized references, interpreting laboratory data, and reviewing patient–specific demographics.
  • Communicate with prescribers, patients and health care team members for prescription clarification and problem resolution, documenting outcomes in the patient health record.
  • Ensure medications are efficiently and accurately dispensed or compounded and labelled, and that all necessary dispensing records are completed.
  • Collect data for medication use evaluation projects, assist in maintaining inventories, report procedural errors, and participate in quality improvement activities.
  • Participate in clinical drug trials, dispensing medications, reviewing adverse reaction reports, and completing necessary documentation.
  • Implement medication safety initiatives and quality control activities in accordance with established standards and procedures.
  • Attend staff meetings, committees and other programs as required, identifying opportunities to improve processes and procedures.
  • Maintain records for narcotic and controlled drugs in accordance with legal, professional and departmental standards.
  • Prepare reports on a variety of activities, gathering and recording information into approved formats, updating computerized files and spreadsheets.
  • Perform other related duties as assigned.

Qualifications

  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.
  • Broad knowledge of organization policies, procedures and standards of care.
  • Knowledge of evidence‑based clinical pharmacy practice, medication use evaluation, and research processes.
  • Knowledge of drug distribution processes in a hospital setting.
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
  • Strong verbal and written communication skills.
  • Ability to establish and maintain effective working relationships with other health practitioners, patients, family members and the public.
  • Excellent organizational and prioritization skills.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
