Through a large and growing network of Hospital-to-Home programs, our interdisciplinary care teams deliver innovative bundled care pathways designed to improve outcomes, reduce readmissions, and provide a seamless experience for clients and families.As an Occupational Therapist with ICS, you'll be part of an integrated care model where collaboration, visibility, and continuity of care are at the center of everything we do.
