Through a large and growing network of Hospital-to-Home programs, our interdisciplinary care teams deliver innovative bundled care pathways designed to improve outcomes, reduce readmissions, and provide a seamless experience for clients and families.As an Occupational Therapist with ICS, you'll be part of an integrated care model where collaboration, visibility, and continuity of care are at the center of everything we do. Working within one interdisciplinary team and one shared client record, you'll have visibility into the entire client journey and the opportunity to see the impact of your interventions throughout recovery.You'll be responsible for the assessment, treatment, education, and ongoing management of clients requiring Occupational Therapy services, helping older adults maximize independence, safely transition home, and achieve meaningful outcomes in their communities.Why Occupational Therapists Choose Bayshore ICS?Flexible work arrangements available, including Full-Time and Part-Time Employee opportunities, as well as Independent Contractor models.Competitive compensation, including premium assessment visit rates that create additional earning opportunities for employee clinicians.Paid mileage, comprehensive benefits, and Group RRSP participation for eligible employees.Work within a fully integrated interdisciplinary team supporting clients transitioning from hospital to home.Collaborate closely with Occupational Therapists, Physiotherapists, Nurses, Social Workers, Personal Support Workers, Rehabilitation Assistants, and other healthcare professionals.Access a shared client chart, providing visibility into assessments, care plans, progress notes, and client outcomes across the care journey.Enjoy greater continuity of care through a coordinated, team-based approach.Contribute to innovative bundled care models that are helping shape the future of community healthcare in Ontario.Ongoing professional development and opportunities for career growth within a nationally recognized healthcare organization.Whether you're a new graduate or an experienced clinician, you'll be supported by a collaborative interdisciplinary team committed to your success.Clinical ResponsibilitiesComplete comprehensive occupational therapy assessments and develop individualized treatment plans, goals, and discharge recommendations.Conduct ongoing evaluation and modification of treatment plans based on client progress and changing needs.Support clients transitioning from hospital to home through evidence-based interventions that promote safety, independence, and quality of life.Collaborate with interdisciplinary team members to ensure coordinated and seamless care delivery.Communicate assessment findings, recommendations, and education to clients, families, caregivers, and healthcare partners.Maintain accurate and timely clinical documentation in accordance with the standards of the College of Occupational Therapists of Ontario.Provide clinical guidance and direction to support personnel involved in the delivery of Occupational Therapy services as appropriate.Participate in case conferences, care planning discussions, and collaborative decision-making processes.Qualifications:Education & RegistrationMaster's Degree in Occupational Therapy or recognized equivalent.Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).Skills & CompetenciesPassion for supporting older adults and helping clients achieve successful transitions home.Excellent communication, collaboration, and relationship-building skills.Strong clinical reasoning and decision-making abilities.Ability to work independently while thriving within an interdisciplinary team environment.Demonstrated proficiency with electronic documentation systems and secure digital communication.Commitment to lifelong learning and evidence-informed practice.Valid Ontario driver's license and access to reliable transportation.Bayshore is committed to a high quality and safe environment for both our staff and clients/patients.
