About Student Wellbeing
The Student Wellbeing department encompasses a variety of wellbeing and accessibility supports at Toronto Metropolitan University (TMU) that reflect a university community that is fully committed and engaged in developing and sustaining supportive environments that foster wellbeing and academic success. This Student Wellbeing team works in a collaborative, student centred manner to enable student success by developing, implementing and overseeing an integrated portfolio of inclusive health, learning, accessibility and wellbeing supports, programs and services using an anti-racist, health equity approach based on the social determinants of health.

About SHaW
includes the Medical Centre, Centre for Student Development and Counselling (CSDC), and Health Promotion Programs. In SHaW, a comprehensive, holistic model of health is engaged to support student health and wellbeing. The Medical Centre provides health care services on campus including assessments, immunizations, physicals, and more. The CSDC offers free, confidential counselling services provided by a team of counsellors and counselling interns. Using a personalized care model, the team works with students to develop a personal wellbeing plan, and services offered include group and one-on-one care. Finally, Health Promotion shares health tips and strategies to students at TMU and runs the peer support programming.

The Opportunity

The Connected Care Team is a pilot initiative to provide coordinated holistic support to the needs of transgender and non-binary students, and more widely 2SLGBTQ+ students at TMU.

Reporting to the Director, Student Integrated Health & Wellbeing, and as a member of the Connected Care Team (CCT), the Care Pathway Specialist plays a key role in supporting transgender and non-binary (TNB) students, as well as the wider 2SLGBTQ+ student community to connect with services and resources at TMU and in the wider community. As part of an interdisciplinary team in the broader department of Student Integrated Health & Wellbeing ("SHaW") , the CCT Care Pathway Specialist serves as a point of contact for trans and non-binary students looking to engage with health, wellbeing, and student success resources. This role supports at-risk health and wellbeing needs through connection and coordinating transitions within the various student support and community-based services.

This role will work and collaborate with colleagues across Student Wellbeing, and the wider student-facing services across TMU. The CCT Care Pathway Specialist will engage in individual and group-based approaches, as well as participate in service co-design and evaluation.

Qualifications
To help us learn more about you, please provide a cover letter and resume describing how you meet the following required qualifications:

  • Successful completion of a post-secondary degree in the field of social work or related discipline.
  • Minimum of 3 years related experience or equivalent combination of education and experience working in a mental health setting.
  • Thorough understanding of issues relevant to mental health and wellbeing for young adults and adults, including developmental issues, trauma, and social determinants of health.
  • Vulnerable sector check is required.
  • Strong interpersonal and relationship building skills.
  • Strong critical thinking, problem-solving and decision-making skills.
  • Strong oral and written communication skills.
  • Knowledge of the student experience, particularly as it relates to the social determinants of health.
  • Experience working with individuals with diverse and intersecting identities through anti-oppressive and trauma-informed practice.
  • Knowledge of the broader mental health system, including emergency mental health services, procedures, and legislation. Clear understanding of privacy legislation. PHIPA, FIPPA, Mental Health Act, Ontario Human Rights Code, AODA, Regulated Health Professions Act.
  • Demonstrated knowledge of computers (MS office, custom databases, LMS, CRM ) and web/social media based applications.
  • Equivalent experience or a combination of equivalent experience and education may be accepted in place of education.

Experience in the following areas would be considered an asset:

  • Post-secondary setting counselling experience supporting trans and non-binary clients, familiar with the mental health needs of trans and non-binary students.
