Position Summary
As an integral member of the healthcare team the Certified Care Attendant (CCA) fulfills assigned responsibilities related to patients daily routines and fundamental care requirements. Working within the framework of hospital guidelines protocols and the units care delivery approach the CCA executes tasks under the supervision of the Registered Nurse ensuring patients well-being and comfort.

UHNs Reactivation Care Centre (RCC) provides a supportive space for specialized care focused on reactivation for patients who no longer need acute care and are focused on the goal of safely returning to the community whether that be home long-term-care or an alternative home setting.

As part of the Ministry of Health and Ontario Healths commitment to alleviating hospital pressures across the Greater Toronto Area the RCC which UHN operates in partnership with Sinai Health and Trillium Health Partners connects patients with the right care in the right place at the right time.

UHNs RCC is nestled in a residential neighbourhood where quiet surroundings and green space extend our calm and healing environment. While therapeutic programming keeps patients focused on achieving their individual reactivation goals whether its building strength and endurance managing medication or maintaining mobility as they plan for a safe transition our recreational schedule builds community encourages socialization and connects patients to activities that are meaningful in their everyday lives.

Providing Exceptional Care
Our dedicated and diverse team of health care providers work together to support patients in realizing their reactivation goals. Together they are committed to putting the needs of patients first and offering an environment where patients feel safe motivated and prepared as they plan for the next step in their care journey.

Duties

  • Follows KARDEX for patient care needs including communication meal assistance transferring ambulation toileting hygiene and grooming.
  • Executes individualized care plans addressing instructions such as diet restrictions safe transferring pressure ulcer prevention restraint use fall prevention and assisted communication.
  • Reports patient condition changes to the Registered Nurse promptly and objectively including skin integrity mental status sleep patterns nutritional factors weight bowel functions and behaviors.
  • Assists with psychosocial patient needs responds to call bells answers phones supports healthcare team provides coverage during breaks attends meetings and contributes to a safe environment.
  • Documents care and changes in patient condition following hospital policy using Core Care Summary and Progress Notes.
  • Operates equipment like lifts and wheelchairs ensuring functionality and notifying staff of concerns.
  • Maintains a safe environment by adhering to safety standards identifying and preventing safety issues attending safety meetings and using personal protective equipment.
  • Exhibits the Centers core values and contributes to patients well-being

Qualifications :

  • Valid registration with the College of Nurses of Ontario
  • High school diploma (Grade 12) or equivalent education.
  • Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA)
  • Personal Support Worker Certification obtained from either an Ontario College of Applied Arts and Technology program or an Accredited PSW Educational Program.
  • Experience preferably at least one year of full-time equivalent work experience within the past three years.
  • Excellent verbal and written communication skills with a courteous approach towards the public.
  • Proficiency in working collaboratively as a team member within an interprofessional environment.
  • Capability to follow directions while also being able to work independently.
  • Fundamental computer skills in Microsoft Office (e.g. Word) Internet usage and Outlook Express.
  • Demonstrated satisfactory work performance in the current or previous roles.
  • Basic Cardiac Life Support (BCLS) certificate preferred.
  • Strong attendance and punctuality record.
