Job Description
The Mississauga Halton Diabetes Foot Care program is a regional program providing preventative foot care to high risk patients with diabetes who are facing barriers to accessing fee for service care models. The Chiropodist serves as a both a mentor and a clinician in this regional program providing foot care to high risk patients with diabetes. This position is part of a regional approach to addressing the needs of this population and has the goal of demonstrating a decrease in admissions for diabetes related wound care. The team will include nurses and chiropodists.
The Chiropodist will provide foot care services in the region at various community settings within a geographic area. The Chiropodist’s approach to client care is evidenced based and innovative and utilizes current therapeutic techniques within the determinants of health framework. This position works within an inter-professional team-based model in terms of both the clinical practice and program planning aspects of its responsibilities. A systems perspective and attention to continuous quality improvement are essential to the success of this position.
· Provide general chiropody services
· Mentor and provide clinical instruction to other health professionals in diabetic foot care
· Act as a resource person to staff, students and the community regarding foot care
· Demonstrate competency in chosen profession; consistently demonstrate knowledge and expertise in professional practice and hospital-based standards

Qualifications
· Recognized diploma in Chiropody required
· Current registration with and member in good standing of the College of Chiropodists of Ontario
· Wound care certification from the Canadian Association of Wound Care
· Experience with mentoring and teaching
· Health Care Professionals who have taken the foot care course preferred
· Knowledge of Self-Management Principles
· Experience with providing education in group settings
· Demonstrates excellent organizational skills to effectively prioritize and handle multiple tasks
· Excellent verbal and written communication skills
· Proven ability to effectively interact within a multi-disciplinary team
· Must have valid driver’s license and access to own vehicle
· Must be willing to travel to multiple foot care sites within Halton and surrounding areas
· Must be physically able to do the job
· Excellent employment and attendance record
