About the Complex Continuing Care (CCC) Unit

The unit is a 19-bed inpatient unit supporting children and youth from infancy to 18 years of age who present with complex medical needs or multi-system diseases requiring ongoing specialized assessment, observation, and intervention. The CCC team provides expert, interprofessional care for conditions including:

• Spinal cord disorders with respiratory insufficiency
• Congenital or acquired airway and lung disease
• Genetic and metabolic conditions
• Neuromuscular disorders
• Skeletal dysplasias

The unit also conducts Sleep Assessments, offering a comprehensive continuum of care for medically complex pediatric clients and their families.

Position Summary

The Social Worker plays a vital role in supporting clients and families as they navigate complex rehabilitation admissions. The successful candidate will complete strengths-based, client-centred psychosocial assessments, provide counselling, and support system navigation. This role also involves coordinating community resources and playing a key role in discharge planning, which may include transitions to home, group homes, community services, or adult services. Guided by Holland Bloorview’s values of equity, excellence, and compassionate care, the Social Worker works collaboratively to optimize outcomes for clients and families.

Key Responsibilities

• Conducts strengths-based biopsychosocial assessments for clients and families of diverse backgrounds who are preparing to be discharged into to community
• In collaboration with the interdisciplinary team, support clients and families to enhance coping and advocacy skills to support the discharge to community
• Identifies need for social work intervention through case screening and case finding methods
• Provides counselling to clients utilizing evidence-based techniques based on client’s presenting mental health concerns
• Initiates and coordinates referrals and support system navigation with regards to funding, community resources and other healthcare services to enable a successful discharge
• Bases practice on established Holland Bloorview policies, procedures, evidence and best practice
• Adheres to Holland Bloorview’s Documentation Policy
• Documents accurate workload statistics
• Attends and participates in program and professional staff meetings
• Communicates in a timely and effective manner
• Contributes to the development of program/service activities
• Ensures the maintenance of the OCSWSSW Standards of Practice and Code of Ethics for Social Workers
• Flexibility to work across other inpatient units
• Identifies learning needs and establishes annual goals
• Remains current with professional literature and best practice guidelines
• Shares new knowledge and best practices with colleagues, staff, students and the community
• Participates in quality improvement initiatives, contributes to education and research activities, and upholds a safe and healthy work environment that fosters a culture of client and patient safety in daily practice.

Qualifications

• MSW Degree from an accredited University, Faculty of Social Work
• Current member of the Ontario College of Social Workers and Social Service Workers
• Current member of the Ontario Association of Social Workers preferred
• Paediatric Social Work or complex care experience in hospital preferred with a specific focus on complex discharge planning
• Previous experience working with youth with disabilities and their families is an asset
• Demonstrated knowledge of common psychotherapy frameworks such as ACT, CBT, DBT and Solution-Focused Therapy and an ability to apply them in session as appropriate
• Experience working in an interdisciplinary setting preferred
• Works effectively as a member of the interdisciplinary team
• Demonstrates ability to manage and respond to crisis calmly and effectively
• Demonstrates ability to organize time and meet deadlines
• Demonstrates initiative and self direction
