Job Summary:
Baycrest Hospital Pharmacy Services support all inpatient units and programs, ensuring the safe and effective use of medications across the organization. The department’s key functions include purchasing and inventory management of medications, management of the unit dose drug distribution system, and operation of a centralized intravenous admixture program. The hospital uses an integrated computerized information system (Meditech) to optimize medication management.
Pharmacists at Baycrest are integral members of the healthcare team, and work within and in collaboration with interprofessional teams, contributing to clinical activities such as medication reconciliation, interprofessional team rounds, medication reviews, medication consultations, problem solving medication adherence issues within scope and mentoring students through preceptorships. The hospital maintains strong affiliation with universities and colleges, offering valuable on-site practical training opportunities for pharmacy students.
This position is an exciting combined position that provides direct patient care & pharmacy services to designated programs and to assume the role of the Clinical Informatics Pharmacist.
Responsibilities include but are not limited to:
- Participates in relevant rounds and care conferences to ensure safe and appropriate prescribing decisions are made, patient outcomes are monitored and adverse drug events are prevented
- Identifies, prevents, and resolves drug-related problems by conducting medication reviews, and developing and monitoring patient care plans. Communicates patient care plans to the interprofessional team, patient, family and/or Substitute Decision Maker
- Ensures continuity of care by conducting medication reconciliation when patients are transferred to or discharged from Baycrest
- Ensures appropriate and accurate preparation and dispensing of medications to designated hospital programs
- Participates in departmental medication management projects and pharmacy clinical programs
- Provides education to pharmacy students as per the requirements of the Faculties of Pharmacy (University of Toronto and University of Waterloo)
- Provides oversight and direction for pharmacy technician(s)
- Ensures best practices are implemented and maintained to enhance medication safety and patient safety
Responsibilities as Clinical Informatics Pharmacist:
- Oversee the maintenance and continuous improvement of pharmacy automation and the Health Information System, including drug dictionary management, testing of interfaces, and system upgrades.
- Collaborate with external vendors to secure technical support and ensure timely resolution of outstanding system issues.
- Identify and assess the impact of informatics interventions, and execute system improvement plans to enhance operational efficiency.
- Conduct audits and report on key metrics to reinforce patient and medication safety
- Serve as the subject matter expert and provide education for Health Information activities for clinical teams
- Participate in departmental and hospital-wide committees as assigned by pharmacy leadership
- Support drug research projects, and assist with the integration of clinical care with research initiatives.
Qualifications include but are not limited to:
- Current license to practice as a Pharmacist from the Ontario College of Pharmacists (Part A)
- Minimum of 2 years of hospital pharmacy experience
- Hospital Pharmacy Residency from an accredited institution or postgraduate Doctor of Pharmacy Degree preferred
- Experience in Health Information Systems management, ie Meditech
- Demonstrated ability to work as an effective member of the interdisciplinary team, and navigate multiple stakeholders
- Demonstrate comprehensive understanding of Electronic Health Records and integration with Pharmacy automation
- Ability to interact with individuals at all levels of the organization
- Well-developed critical appraisal and critical thinking skills, and an approach to work that is detail oriented and analytical
- Proficient with Microsoft Office programs
- Self-motivation, excellent communication and organization skills required
