Job descriptionInitiate intake interviews and conduct needs assessments; Work in social rehab area, provide food, ensure residents are gathered, maintain safety / security, follow escalation and food protocols, brief on-site training provided Develop and implement a plan of care, with short and long-term objectives and monitor the effectiveness of the plan and the need for revision; Provide case management coordination which includes advocacy, coaching, risk assessments, crisis prevention / intervention, referrals to primary care, financial aid, assisted devices, home and safety assessment resources, food banks / meals-on-wheels, transportation and social recreational programs, etc.; College Diploma / University degree in social services or related field, BSW or MSW an asset; Minimum 2 years’ experience, mental health / social services background Knowledge of seniors issues and services; Experience working with clients with age related mental and / or physical health issues. Excellent verbal and written communications skills; Demonstrated case management and referral skills; and ability to effectively link clients to supportive services; Self-directed and independent, but also experienced in working effectively as a member of multi-disciplinary team, which includes internal and external stakeholders; Applied computer skills (Microsoft Office, Client Information Systems, Web browsing); Do you have a knowledge about the software, Procura? Do you have Valid Driver’s license and access to a personal vehicle as its required? Are you comfortable to travel within the city during your working hours? Master's Degree (preferred) in seniors case management and care coordination support : 3 years (preferred) 75% (preferred)
Provide outreach and advocacy services, counseling and care planning to individuals and caregivers directly and through support group leadership. In addition, the Supervisor will be responsible for community development, assistance in acquiring program funding and consultation with stakeholders.
