- Provides psychosocial assessments, crisis intervention, discharge planning and counseling to patients and their families, including advanced care planning and end-of-life care
- Provides support / consultation to other disciplines on the team re : psychosocial issues particularly related to complex patient / family situations
- Acts as the team expert on discharge planning and patient flow
- Acts as a linkage and communication conduit between the family and the care team
- Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services
- Participates in teaching and other student supervision activities for health professional students assigned to this unit
Qualifications include but are not limited to :
- Masters of Social Work degree
- Current and valid registration with the Ontario College of Social Workers and Social Service Workers ( OCSWSSW’)
- Experience in discharge planning and system navigation preferred
- Experience working in a healthcare setting preferred
- Excellent interpersonal, communication and problem-solving skills
- Computer skills Microsoft office software including Excel, Word, PowerPoint and outlook.
