About the role

As Regional Primary Care Coordinator, you will assist the Primary Care Manager and Primary Care Project Manager in the delivery of culturally safe, efficient, and quality Primary Care Services.

The Primary Care Coordinator provides both operational and administrative support for the coordination and implementation of primary care services and initiatives with Northern First Nations communities, including, but not limited to :

  • Primary health care services (rural, urban and isolated)
  • Program planning, development, implementation and evaluation
  • Primary Care Network Initiatives
  • Joint Project Board Initiatives (mPCT)
  • Integration and Partnership initiatives with health partners (Divisions of Family Practice, Northern Health

Authority, First Nations Health Service Organizations)

Integration efforts of primary care with FNHA’s Mental Wellness and Addiction Teams.

About you

  • Bachelor’s degree or an acceptable combination of education, training and experience
  • Three (3) years or more related experience, including a previous Coordinator or Administrative role, or an equivalent combination of experience and education.
  • Recent, relevant experience in a health care environment
  • Experience working with First Nations communities or organizations including political leadership
  • Excellent interpersonal and communication skills; ability to foster collaborative partnerships utilizing a team approach
  • High level motivation with a positive attitude and high degree of flexibility in a rapidly changing environment
  • Strong organizational skills and experience in project coordination; ability to organize and prioritize work and balance multiple activities concurrently
  • Strong computer skills and ability to work comfortably with spreadsheets, presentations, diagramming and word processing software (e.

g. Microsoft excel, PowerPoint, Visio and Word)

Experience and knowledge in Regional Health Authority and First Nations Primary Care and virtual health processes, workflows, referral patterns, program delivery models and electronic medical records
