Job description

Position Overview

Case management involves coordinating care to meet the needs and wishes of individuals with chronic and / or complex illnesses and / or those at high risk of criminalization or victimization.

The case manager’s main responsibilities include conducting psychosocial assessments; coordinating multidisciplinary care; advocating for the patient; liaising with external agencies; sharing the care plan with the treatment team; and engaging relatives and families.

TEMPORARY PART-TIME POSTION 0.4 FTE for undetermined period

HGH is a bilingual community hospital which has more than 800 employees and 160 physicians, and serves the Prescott-Russell region in Ontario.

What we are looking for

  • Bachelors in social work.
  • Member in good standing with a professional college in Ontario.
  • Minimum of three (3) years of experience in mental health and addictions / gambling and concurrent disorders.
  • Be able to plan and coordinate care;
  • Be able to complete the liaison and service transition;
  • Have a thorough knowledge of the social context in a defined area;
  • Have a thorough knowledge of the functions and roles of the different care providers;
  • Have a good understanding of the limitations of the system's services;
  • Be able to build a quality relationship with the patient, family caregivers and caregivers;
  • Be able to empower patients
  • Ability to work in a multidisciplinary environment and inter-agencies
  • Possess a working knowledge of both official languages.
