• Assesses the dietary end-of-life needs unique to each person and family, considering the social determinants of health, ethnicity, culture, gender, sexual orientation, language, religion, age, ability and their preferences.
• Assesses the person’s and caregivers’ skills and need for dietary education and supports
• Creates a holistic, person-centred plan, acknowledging the psychosocial impact of changing nutritional requirements and dietary intake
• Provides education, advice and practical strategies to the person and caregivers regarding the management of dietary related symptoms, including anorexia, cachexia, dysphagia, nausea, vomiting, constipation, diarrhea and taste changes, management of enteral and parenteral nutrition support
• Engages in dysphagia assessment and management
• Collaborate with interdisciplinary teams, including but not limited to physicians, nurse practitioners, nurses, personal support workers, social workers, speech therapists, occupational therapists, and physiotherapists.
• Contributes to interdisciplinary team meetings and participates in care planning conferences
