Your Opportunity:
Ponoka is located along Highway 2 in the Battle River Valley within an hour drive to major cities. Our community provides a great opportunity for families, adventurists, and people wanting a rural life. The Ponoka Home Care program serves a large area of clients including the community of Ponoka and surrounding areas. We work closely with community groups, other health care services in the community and as part of a multidisciplinary team. This is a Registered Nurse - Case Manager position for the Type B/SL case load. The Case Manager must have strong interpersonal communication and interdisciplinary team player skills, excellent critical thinking, creativity, resourcefulness, time management, and organization abilities. The Case Manager responsibilities include assessment of client needs, establishing goals for client care delivery, planning, delivering, and coordination of care needs, resources, and services within professional practice standards and home care program guidelines, communication and coordination of client care with the interdisciplinary team; and evaluation of outcomes. New grads will be considered and training is available for the right candidate.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $ - $ per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii) as applicable.
Required Qualifications:Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): NextGenU Health Assessment Resources Certification:
Additional Required Qualifications: Preferred Qualifications:Experience with Connect Care, Care Manager, Microsoft Office and general computer skills.
