About Us:

Since 1988, Pozitive Pathways, a registered charity, has evolved and diversified to address the emerging needs of our community. Our staff, peers, and volunteers provide in-reach and out-reach services to clients and service users through our Support Services and Needle Syringe Programs. Additionally, we provide education and resources to community members and service providers working with populations that are most vulnerable to HIV and HCV transmission.

About the Role:

The Harm Reduction Outreach Coordinator contributes to the program’s goals of reducing the incidence of Hepatitis C (HCV) and HIV transmission among Person’s Who Use Drugs (PWUDs). The offer of employment is for a full-time position (35 hours per week) and the execution of work within Windsor-Essex and Chatham-Kent.

Responsibilities:

  • Work as a member of the Harm Reduction Department to implement the annual workplan and budget in line with the agency’s strategic plan and provincial Harm Reduction objectives.
  • Represent the agency during provincial and regional meetings.
  • Maintain collaborative relationships with community partners and colleagues that serve or could be in line to serve PWUDs.
  • Participate on local committees to help add value and collaborate with other service providers in the region to strengthen service integration and increase service accessibility for PWUDs.
  • Engage PWUDs in education at community agencies and partners to reduce instances of harm by increasing uptake of sterile supplies.
  • Implement and provide oversight of the Service Provider NSP Training Program.
  • Increase accessibility of Needle Syringe Program (NSP) services throughout Windsor-Essex and Chatham-Kent by:

o Assisting with oversight of existing NSP sites and other partner organizations.
o Assisting with the facilitation of NSP partnership meetings.
o Assisting with assessing the feasibility of potential new NSP satellite sites.
o Assisting with the onboarding of new NSP satellite sites.
o Assisting with the evaluation of service delivery model.

  • Participate in local events to increase awareness about harm reduction.
  • Outreach at regional shelters/transitional housing hubs to distribute harm reduction supplies, pre-packaged kits, and associated paraphernalia to PWUDs.
  • Ensure data (service user code, # of resources distributed, education, etc.) from all outreach activities are documented/tracked.
  • Refer PWUDs seeking services over and above those offered through the NSP to local community resources.
  • Incorporate persons with lived/living experience (peers) into program activities (where applicable).
  • Contribute content for the agency newsletter, The Pozitive Post.
  • Responsible for the maintenance of NSP information boards.
  • Adhere to current Pozitive Pathways policies and procedures.
  • Participate as an active member at all department and agency staff meetings.
  • Other related duties as assigned by the Director of Programs & Services or designate.

Requirements:

  • Minimum 1–2 years’ experience working in harm reduction, outreach, addictions, public health, or community-based social services.
  • Knowledge of commonly used illicit substances and their associated risks, trends, and harm reduction strategies.
  • Ability to build and maintain collaborative relationships with community partners and service providers.
  • Strong documentation and data tracking skills, with attention to accuracy and confidentiality.
  • Proficiency in Microsoft Excel, Word, Outlook, and Teams software.
  • Excellent verbal and written communication skills (including ability to contribute to newsletters or written materials).
