We are searching foran Occupational Therapist to join our Transitional Health Centre located in West Bedford, Nova Scotia.
The Occupational Therapist is integral in supporting the goal of returning to home / community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient / family goals, using effective communication strategies, shared decision-making, and mutual support.
The Occupational Therapist supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.
The transitional care model supports Nova Scotia Health patients / families who require additional time, services, and support to transition back to home / community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation. It all began in when our Founder, Joseph Shannon, purchased a single nursing home in his hometown of Sydney, Cape Breton. For 35 years, Shannex has grown as a trusted provider of senior accommodations, services and care in Nova Scotia, New Brunswick, and Ontario. Through a new, innovative partnership with Nova Scotia Health, we will open the first transitional care center in our province, in December , in West Bedford. Shannex is the operating partner for the transitional care centre on behalf of Nova Scotia Health. Occupational Therapists will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services like occupational therapy before they can safely return home / community.
