Joignez-vous aux Programmes de soutien aux patients de McKesson afin d’aider les patients à commencer et à maintenir leurs traitements de spécialité. À titre d’infirmier(ère) gestionnaire de cas bilingue, vous agirez comme point de contact unique pour les prescripteurs, le personnel clinique et les patients, en coordonnant l’accueil, l’éducation et le soutien à l’adhésion tout au long du parcours thérapeutique. Ce rôle hybride (terrain/télétravail) comprend des déplacements vers des cliniques, des domiciles ou la tenue de visites virtuelles afin d’offrir de la formation sur l’injection, de l’éducation sur la maladie et les médicaments, et de collaborer avec les équipes de soins pour assurer un accès fluide et rapide au traitement.

Vos responsabilités :

  • Agir comme point de contact principal pour les prescripteurs et les équipes cliniques afin d’inscrire les patients et de coordonner les services.
  • Offrir de l’éducation aux patients (maladie, médication, gestion des effets secondaires) et dispenser de la formation sur l’injection à domicile, en clinique ou en mode virtuel, selon les préférences du patient.
  • Soutenir l’adhésion au traitement par des suivis programmés et transmettre en temps opportun les mises à jour cliniques aux médecins et au personnel clinique.
  • Effectuer une documentation concise et précise, ainsi que des révisions de dossiers au besoin dans le cadre du soutien au remboursement.
  • Collaborer avec les collègues des pharmacies McKesson, les partenaires manufacturiers et les équipes cliniques afin d’éliminer les obstacles aux soins et d’escalader les enjeux dépassant le cadre du programme, au besoin.

Exigences de base :

  • Bilinguisme : Maîtrise du français et de l’anglais, à l’oral et à l’écrit.
  • Permis d’exercice infirmier en Ontario ou au Québec (ou les deux). Le titre d’infirmier(ère) autorisé(e) (IA) est fortement souhaité; les infirmier(ère)s auxiliaires autorisé(e)s (IAA/LPN) et les infirmier(ère)s praticien(ne)s (IPS) seront considérés.
  • Minimum de 2 ans d’expérience pertinente (ou une combinaison équivalente de formation et d’expérience).

Compétences et expérience souhaitées :

  • Expérience directe en gestion de cas, en éducation des patients ou en soutien aux thérapies de spécialité.
  • Expérience en immunologie, rhumatologie, dermatologie ou gastroentérologie; connaissance des produits biologiques et des évaluations préthérapeutiques (p. ex. tests de dépistage de la tuberculose, imagerie, analyses de laboratoire).
  • Aisance à offrir de la formation sur l’injection et à utiliser les principes de l’apprentissage des adultes pour adapter l’enseignement.
  • Excellentes compétences en établissement de relations, en documentation et en utilisation de la suite Microsoft Office; capacité à prioriser efficacement dans un environnement de travail sur le terrain comportant de multiples intervenants.

Exigences physiques :

  • Territoire couvert : Québec et/ou grande région de Toronto (GTA).
  • Les heures du programme sont de 8 h à 20 h (HE).
  • Les heures de travail sont de 9 h à 17 h (HE), du lundi au vendredi (sujettes à changement).
  • Des déplacements occasionnels peuvent être requis.

About the Role:

Join McKesson’s Patient Support Programs to help patients start and stay on specialty therapies. As a Bilingual Nurse Case Manager, you’ll serve as the single point of contact for prescribers, clinic staff, and patients—coordinating onboarding, education, and ongoing adherence support across the treatment journey. This field/remote role includes travel to clinics, homes, or virtual visits to deliver injection training, disease/medication education, and to liaise with care teams for smooth, timely access to therapy.

What You’ll Do:

  • Act as the primary contact for prescribers and clinic teams to enroll patients and coordinate services.
  • Provide patient education (disease state, medication, side‑effect management) and deliver in‑home, in‑clinic, or virtual injection training based on patient preference.
  • Support adherence through scheduled follow‑ups; share timely clinical updates with physicians/clinic staff.
  • Complete concise, accurate documentation; conduct chart reviews as needed for reimbursement navigation.
  • Collaborate with McKesson pharmacy colleagues, manufacturer partners, and clinic teams to remove barriers to care and escalate issues beyond program scope when required.

Basic Requirements:

  • Bilingual: Fluent in French and English (spoken and written).
  • Nursing license in Ontario or Quebec (or both). RN strongly preferred; RPN/LPN and NP considered.
  • 2+ years of relevant experience (or an equivalent combination).

Preferred Skills/Experience:

  • Direct experience in case management, patient education, or specialty therapy support.
  • Experience in immunology, rheumatology, dermatology, or gastroenterology; familiarity with biologics and pre‑treatment work‑ups (e.g., TB testing, imaging, labs).
  • Comfortable delivering injection training and using adult‑learning principles to tailor teaching.
  • Strong relationship‑building, documentation, and MS Office skills; ability to prioritize across multiple stakeholders in a field‑based environment.

Physical Requirements:

  • Territory location: Quebec and/or Greater Toronto Area (GTA).
  • Program hours are 8am to 8pm EST.
  • Job hours are: 9-5PM EST, Monday to Friday (subject to change)
  • Occasional travel may be required.
