What will you work on?
The Patient Care Coordinator (PCC) practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model. The PCC is responsible for the coordination of day-to-day operations and quality patient care within a designated clinical area(s). This position supports the seamless movement of patients through the healthcare system and in accessing appropriate resources. The PCC supervises as well as serves as a role model and a resource to staff for clinical decision-making, clinical assessments of patients and in the counseling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of patient care, adequate staffing levels, and a continuous quality improvement environment. Liaises with the interdisciplinary team for identification, coordination and efficient utilization of resources as well as plans for staff educational needs.
