about the role :
Sherbourne Health has been funding by Immigration, Refugee and Citizenship Canada (IRCC) to provider our Finding Health Project.
Our project is dedicated to delivering services at the intersection of health and settlement. We are seeking a committed Case Manager to join our team and assist newcomers in navigating the healthcare and settlement systems in Canada. This role offers a unique opportunity to make a significant impact on the lives of newcomers by facilitating their integration into Canadian society. The Case Manager will work at Sherbourne Health and other locations, including the community hub at St. James Town Community Corner.

The regular hours of this position are Monday to Friday, from 9 : 00 am – 5 : 00 pm; however, this position will necessitate occasional work on evenings and weekends to be available when newcomers and services are available. In addition, this position requires working in a fast-paced environment meeting the needs of multiple stakeholders. Therefore, time management, multi-tasking, initiative, organization and prioritizing are essential.
