Speech Language Pathologist Consultant - Temporary Part-Time (0.5 FTE)
- The primary functions of the Speech-Language Pathologist Consultant include : assessing patients who are referred for SLP services;
- developing swallowing and / or communication plans of care for the patient; collaborating with the interprofessional care team to implement and evaluate the plan of care;
and education of patients and / or their caregivers.
Responsibilities include :
- Provides assessment, management and rehabilitation of swallowing disorders for patients admitted to Runnymede and referred for SLP services.
- Provides assessment, management and rehabilitation of speech, language and cognitive communication disorders for patients admitted to Runnymede and referred for SLP services.
- Develops and implements individual plans of care based on an analysis of assessment findings and clinical reasoning.
- Establishes rehabilitation goals in collaboration with the patient and / or substitute decision maker.
- Monitors changes in the patient’s functional status related to swallowing and communication and adjust plans of care accordingly.
- Provides education to patients, families and caregivers as required to support optimal and safe care for swallowing and communication disorders.
- Contributes to discharge planning, including initiating referrals for community services as appropriate, for patients on their caseload, when indicated by the plan of care.
- Collaborates with members of the inter-professional team through documentation and / or direct consultation to provide a coordinated program of patient care.
- Maintains accurate and up-to-date assessment, treatment, goals and other relevant documentation for each patient on their caseload.
- Educates patients, appropriate family members, students and other health care providers regarding aspects of communication and swallowing disorders.
- Complies with College Standards, Guidelines, Position Statements and Code of Ethics.
- Adheres to Runnymede’s vision, mission and values.
- Actively promotes a culture of safety for both patients and employees by participating in safety education and training, by reporting incidents or unsafe situations to his / her supervisor, and by complying with the Occupational Health and Safety Program requirements.
The successful candidate will possess the following qualifications / experience :
- Master’s Degree in Speech Language Pathology from an accredited and recognized university
- Member in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario
- Membership with the Canadian Association of Speech-Language Pathologists and Audiologists an asset
- Basic Cardiac Life Support Certification
- Demonstrated expertise with geriatric, neurological and MSK conditions
- Clinical experience in Rehabilitation / Complex Continuing Care Hospital or Acute Care Hospital preferred
- Effective verbal and written communication to successfully work with all levels of the organization
- Competence with the Windows Operating System and Microsoft Office Suite
