Position Details
This position will appeal to OTs seeking a diverse community-based practice serving clients across the rehabilitation continuum. Duties include :
- Hospital discharge planning and assessments
- Cognitive assessments & rehab to increase client independence
- Graduated activity plans to increase home / community functioning
- Development & implementation of community integration plans
- Case management to facilitate client access to other health services
- Development and supervision of rehab assistant plans
- Liaising with funders, family and healthcare team
