Emotional Well-being and Mental Health Nurse - Clinical Lead (EWMH)
Job summary
This service covers an enhanced model of care to provide wrap-around care for children and young people in our acute hospitals and 136 suites for Surrey young people, 12 hours a day, 7 days a week.
The team builds relationships within the acute hospitals to further support care for the young person holistically (emotional, mental, and physical) and to improve communication between the teams involved.
As part of the EWMH Team, you will be ensuring that the physical health and mental health needs of children and young people admitted to the district general hospitals are being met in order to reduce the crisis presentation.
Facilitate timely discharge to community services with appropriate follow-up or support until such time as a T4 bed or social care placement is in place.
Plan and deliver interface meetings with appropriate community services. Upskilling team members, including the paediatric team on the wards, regarding mental health, disordered eating, neurodevelopmental, and emotional health issues.
This is an 18-month fixed-term contract that may be extended with the success of the pilot service. There will also be opportunities within our crisis pathways at the end of this term.
Main duties of the job
- To actively contribute to the development and implementation of the service
- To provide leadership to staff within the team
- To maintain accurate and timely statistical information as required by senior staff and managers.
- To ensure that all accidents, incidents, near misses and complaints are appropriately managed and reported
- To lead and clinically manage children and young people who require increased support with assessing and monitoring their physical health care needs and their mental health needs this may include disordered eating due to neurodevelopmental issues or challenging behaviours.
You will lead the interface with specialist community services, including safeguarding teams, to ensure a holistic approach to meeting concerns is being taken.
- Responsible for liaising with medical colleagues regarding investigations and can make referrals on to an appropriate service provider when required
- To offer support, guidance, and training to the team so that they can appropriately identify presentations of children and young people and what may be helpful to meet their needs during their stay
- Plan, develop, and provide training and education for people using services, caregivers, and staff teams at different levels according to the need.
- Provide reflective spaces for the staff on the ward to explore the emotional impact and well-being following any challenging presentations by children and young people on the ward.

Job description
Job responsibilities
Ensuring that the physical health and mental health needs of children and young people admitted to the paediatric ward are being met in order to reduce the crisis presentation and facilitate timely discharge to community services or support and care for children and young people while they wait for a T4 adolescent admission or appropriate social care placement.
You will be responsible for providing assessments and care planning that relate to the presentation of children and young people in conjunction with the paediatric teams in the acute hospitals.
You will be leading a team of Band 6 nurses and Band 3 support workers as part of this dedicated service. You will be expected to work on a rota basis, covering days, weekends, and bank holidays.
Working closely with the team, you will support the children and young people presenting with mental health needs, disordered eating, neurodevelopmental, and emotional health issues and provide timely interventions that contribute to their care.
You will also be working alongside acute care paediatric colleagues on the ward and the support workers.

- Evidence of post qualification study

- Undertaking mental health assessments, risk assessments and treatment of CYPS Working as part of a multi-disciplinary team.

Qualifications

- Post graduate in nursing
