Job Summary : Were seeking compionate and dedicated Personal Support Workers (PSW) Health Care Aides (HCA) or Continuing Care istants (CCA) to join our awardwinning team. In this role youll provide direct personal support and care to patients addressing their physical intellectual emotional social and spiritual needs in accordance with our patientcentered policies and procedures.

Key Responsibilities :

Provide personalized care to patients including istance with ambulation feeding bathing and grooming

Implement and follow care plans as directed by nursing staff

Doent patient care activities and observations accurately and promptly

Communicate effectively with patients families and other healthcare team members

Create a safe comfortable and dignified environment for patients Participate in ongoing training and professional development opportunities

Qualifications and Ss :

Completion of a PSW Program or HCA / CCA certification that meets appropriate standards

Experience in Long Term Care or community home care setting (preferred)

Knowledge of dementia and palliative care (preferred)

Familiarity with care planning systems and doentation

Strong communication ss and ability to interact positively with staff residents / patients and families

Proficiency in reading writing and understanding verbal and written instructions in English

Ability to work independently especially in a Community Home Care setting

Access to a reliable vehicle for community home care (required)
