Functions as a member of an empowered nursing team, adhering to professional legislation and College of Nurses' standards.
Organizes and plans patient care based on appointment schedules, nursing requirements, patient preferences, and available clinic space.
Assists in patient bookings and maintains continuity of care.
Restocks clinic rooms and manages medical supplies.
Interfaces with physicians and healthcare professionals for necessary orders.
Utilizes the nursing process for individualized patient care, conducting holistic assessments and goal setting.
Promotes patient safety and participates in related education and reporting.
Following Patient Care Plan
Develops comprehensive care plans reflecting evidence-based practice and patient goals.
Implements care plans, emphasizing fiscal responsibility and patient self-management.
Utilizes teaching skills to empower patients in decision-making and independence.
Evaluates patient response to treatment, revising care plans as needed.
Provides education and precepts new nurses or students as appropriate.
Documentation and Communication
Documents assessments, care plans, and patient responses according to company policies.
Communicates with the LHIN as per standards.
Collaborates with healthcare team members for comprehensive patient care planning.
Participates in meetings, in-services, and professional development activities.
Actively engages in the agency's quality improvement and patient safety programs.
What you must have :
Graduate of an approved Registered Nursing program RN or RPN
Current College of Nurses RN / RPN Certificate of Competence.
Proficient computer skills.
Effective communication in English, both oral and written.
Two years of recent acute care / community-based nursing experience recommended, along with IV initiation and central line experience.
Salary / Rate Range : $50,000 - $60,000
