Social Worker I, Permanent Full-Time
Community Supports, Green Bay Health Centre, Springdale
Definition of work :
This is an advanced professional level position involving a high degree of responsibility, autonomy and independence in identifying and providing specialized assessment, individual service planning and critical intervention services.
Within policy parameters, the Social Worker accepts, screens, and actions referrals in relation to adults who require supportive services, completes comprehensive client assessments and using an interdisciplinary approach, assists the client through service planning based on individual needs and available resources.
Work involves carrying an active caseload of clients in receipt of an array of services and working directly with clients / families / care providers, social workers continually assess and monitor client strengths, resources, needs, and approved services.
Adhering to the Adult Protection Act of Newfoundland and Labrador, the Social Worker accepts and evaluates reports of adult abuse / neglect and conducts investigations when necessary.
Social Worker's also adhere to standards for client services in accordance with the Canadian Association of Social Workers Code of Ethics, the Standards of Practice in Social Work, and the Social Workers Act of Newfoundland and Labrador.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications :
- Bachelor Degree in Social Work.
- Current registration with NLCSW.
- 2 years' experience within the last 5 years working in the Community Supports Program.
- Familiarity with the Provincial Home Support Program and the Long-Term Care and Community Support Program Adult Needs Assessment.
- Familiarity with assessment tools including the Long-Term Care and Community Support Program Adult Needs Assessment and the RAI-hc Assessment.
- Considerable knowledge of the needs, resources and services available to seniors and individuals with disability.
- Demonstrated ability in comprehensive assessment, interviewing and crisis intervention techniques, and social work programming and delivery methods.
- Established analytical capability and oral / written communication skills.
- Ability to work both independently and as part of a team.
- Availability to participate in an on-call rotation.
- Daily use of a private vehicle for employment.

- A satisfactory record of work performance and attendance is required for this position.
- A satisfactory Certificate of Conduct from the RCMP or local police authority.
Hours of Work : 75 hours bi-weekly (Shift work may be required)
Salary : CG-39 ($37.20-$41.63)
