The Continuing Care Home Coordinator uses a collaborative approach to review all applications for Continuing Care Homes, as well as manage the Central Zone wait list according to guidelines and policies in place. The coordinator will also act as a resource for facilities and community health professionals, revise priorities on a continuous basis and coordinate the placement of individuals in Continuing Care Homes in the Central Zone.

Description :

As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.

Required Qualifications :

Active or eligible for registration with applicable professional association or college.
