Your Opportunity :

This position provides a unique opportunity to work to your full scope of practice as the Social Worker on the Red Deer Stroke Early Supported Discharge Team.

The Stroke Early Supported Discharge Team is an interdisciplinary team that facilitates early discharge from hospital for acute stroke clients by providing the option of intensive rehabilitation in the client's home environment.

It is the role of the Social Worker on the ESD team to provide support to clients and their families in their home environments while the client participates in their home rehabilitation program.

The successful candidate will have the ability to develop collaborative partnerships with community service providers in order to ensure successful reintegration into the community.

Experience supporting clients through grief and loss is an asset. Knowledge of community resources and financial supports is required.

Effective time management and excellent written and verbal communication skills are essential.

Description :

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families.

Your services may include : psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications :

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
