Band6 Looked After ChildrensNurse :

JobResponsibilities :

• Toprovide ongoing advice and support to residential care staff withregards to the health needs of LAC and Careleavers.
• To consult with and evaluate LAC andCare Leavers experiences of health services and develop servicesthat are responsive to their needs.
• Work withinthe professional boundaries of confidentiality and consentbalancing the need to share information and safeguard LAC and CareLeavers.
• Ensure all LAC and Care Leavers areprovided with a comprehensive health summary upon leavingcare.
• To maintain full accurate andcontemporaneous electronic health records in line with the TrustRecord Management Policy.
• To be responsible forundertaking and coordinating health reviews for LAC using agreedhealth assessment tools in order to improve LAC health outcomes& access.
• Working together with CYP toexplore the best ways to engage them and meet their health needsprovided within the context of the core values of the Trust forcompassionate care and respect for all.
• Supportthe effective implementation of transition protocols for LAC andthose leaving care moving from childhood through toadulthood.
• Provide telephone / helpline advice tochildren and young people those with parental responsibility fostercarers and residential social workers;

acknowledging consent issuesappropriate within local and national legislation orguidance.

• Provide dropin sessions to childrenand young people as appropriate & contribute to multiprofessional support days.
• Provide a liaisonrole that offers sign posting child specific liaison and support toaccess other services such as Child and Adolescent Mental HealthServices CAMHSLAC team Substance abuse Dental and Integrated SexualHealth Services (ISHS) and the CSE NurseSpecialist.
• Ensure LAC CYP are givenopportunities to have time alone to express any concerns orpersonal issues to support their health safeguarding &social care needs;

particularly during healthassessments.

• To play a key role in increasingthe immunisation uptake for Looked afterChildren.
• To travel to other boroughs asrequired to ensure effective assessment and implementation ofhealth care plans.
• To use RiO in casemanagement of children and to maintain an overview of thecaseload.
