Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients / clients / residents and families : Employing a strong focus on service and community integration and working closely with multiple program leadership teams and internal / external stakeholders, the Director, Clinical Operations & Planning, Long Term Care Community & Initiatives, is accountable to lead the development, re-development and oversight of Fraser Health Long Term Care Communities.
Reporting to the Executive Director, provides leadership in creating and implementing new models of service to optimize resident experience and systems efficiency, including identifying service integration opportunities across the continuum.
Responsible for the regional overview of effective operation and management of new long-term care services which includes service planning, implementation, operation and evaluation of sustainable projects and initiatives that facilitate and contribute to the transformation and integration across services.

Responsible for the Fraser Health wide overview of effective operation and management of new long-term care services which includes service planning, implementation, operation and evaluation of sustainable projects and initiatives that facilitate and contribute to the transformation and integration across services.
Responsible for staff management, development, and performance including corresponding budgetary accountability including coordinating internal / external consultant / teams as appropriate for clinical services planning of Fraser Health long-term care services.
Responsibilities In collaboration and partnership with Executive Directors and operational directors, the Director is accountable to plan, guide and facilitate the implementation of initiatives, programs and services related to new long-term care development projects.
Scope includes the planning, operation, delivery and evaluation of services and programs to facilitate the transformation and integration based on evidence-based clinical practices for long-term care residents and staff.
In collaboration and partnership with operational leadership, ensures the development of assigned regional services and the effective management of long-term care.
Scope of accountability includes the evaluation of initiatives, operational activities and the assessment of expected and / or pre-determined outcomes, ensuring alignment with approved financial mandate.
Assists the Executive Director and operational Directors in the development of long-term care services by preparing clinical program descriptions and participating in the assessment, redesign and / or formulation of operational systems including assessing logistics, financial, and equipment requirements.
Provides leadership and oversight related to long-term care through collaboration and partnership with operational leadership.
Leads the design, planning, implementation and integration of service delivery models for regional services consistent with the strategic and operational goals of Fraser Health's broader long-term care services strategy.
Works closely with local clinical and operations teams, physicians, Primary Care Networks, the Division of Family Practice, clinical leaders and other health care professionals to identify and plan for present and future program and service requirements and to review and integrate service delivery models, clinical standards and operational guideline for assigned clinical services within long-term care.
Provides guidance and direction on process improvement strategies that cross service areas; facilitates task groups as required and communicates status of relevant changes of outcomes to relevant user groups;
integrates and coordinates resource planning for long-term care development. Provides leadership, planning and supports the development of integrated multi-disciplinary teams of health professionals.
Leads the evaluation of integrated teams on an ongoing basis during development phases. Strengthens care coordination and management of transitions between acute, community and long-term care.
Serves as high-level contact / resource person for long-term care regarding development / re-development projects and management of issues.
Liaises with provincial committees and representatives; ensures the monitoring, coordination, management and evaluation of long-term care projects within the project management framework, including required documentation and communications.
Ensures the effective recruitment, management and development of employees for new and existing program areas; accountable for effective development and management of staffing budgets;
reviews and endorses budgets developed by clinical operations and implements the same regionally. Develops regular communication tools such as project activity reports, measurement indicators, and service reports as defined by stakeholders to ensure awareness of service delivery goals, key performance indicators, and effectiveness results.
Represents FH regarding long-term care development on committees, organizations, and public relations activities, as appropriate, at local, provincial and national levels as requested.
Qualifications Master’s Degree in Health, Business Administration or related health care discipline which includes clinical background and / or knowledge, plus ten years recent progressive leadership / management experience in complex health care project / program planning, development, integration and / or redesign which includes five years related clinical and operational experience;
or an equivalent combination of education, training and experience. COMPETENCIES : Demonstrates the leadership practices of the Fraser Health Leadership Framework of Clear, Caring and Courageous and creates the conditions for people to succeed.
Professional / Technical Capabilities Comprehensive knowledge of long-term care, and community care services. Extensive knowledge of clinical operations and service planning for clinical, non-clinical, and support services.
Demonstrated implementation of complex system-wide projects that requires collaboration and working with interdisciplinary groups.
Expert knowledge of strategic planning, change and project management principles coupled with a comprehensive understand of the he healthcare system at both the strategic and operational levels.
Knowledge of the delivery of care in a clinical operations system, government processes, health authority structures and responsibilities.
Demonstrated ability to prioritize multiple requests and meet tight deadlines in a dynamic environment. Demonstrated ability to problem solve, persuade, analyze and implement.
Demonstrated analytical, statistical, research and indicator development skills.
