The Nurse Practitioner, CMH Clinic, will function in aninterdisciplinary team environment. The role will primarily focus
upon the management of patients with an oncology / hematologydiagnosis.
The NP has a direct reporting relationship to the Program / ServiceDirector and matrix relationship for nursing
practice to the Director of Nursing. NPs will have a professionalaccountability for quality and safety to the Medical
Advisory Committee through the Chief of Department and matrixrelationship to the Chief Nurse Executive. The NP
is an expert practitioner with a specialty based focus whofunctions with a high level of autonomy, knowledge and
skills to formulate clinical decisions and manage chronic illnessthrough an interdisciplinary team approach. NP
practice addresses the five domains of Advanced Practice Nursing : Clinical Practice, Collaboration, Research,
Leadership, and Change Agent. The percentage of time dedicated toeach of these domains is variable, based on
the program needs and will evolve with the program.
Primary Responsibilities :
The NP's practice is primarily focused on caring for patients withan oncology / hematology diagnosis, and could also
involve a broad range of chronic disease conditions. Skillsinclude : advanced physical assessment, clinical
reasoning, pharmacology and collaboration with physicians and theinterdisciplinary team to formulate effective care
plans. Patient education and ongoing emotional support are keyelements required for success in the chronic
disease population.
Must provide patient care or services in a manner that isconsistent with TBRHSC mission and reflects the core
values of Patient & Family Centred Care.
Must treat all information concerning patient affairs orillnesses and / or hospital business as confidential, privileged
information, which must never be discussed except in the course ofduty.
Must be accountable for ensuring the safest patient care andservices and for recognizing and reporting safety
incidents or potentially unsafe conditions.
Employment Requirements :
- Unencumbered registration as a Registered Nurse in the ExtendedClass with the College of Nurses of Ontario.
- Nurse Practitioner certification in Primary Health Care orAdult streams.
- Membership in RNAO or NPAO is required.
- Successful completion of Graduate Studies.
- Minimum 3 years recent nurse practitioner experience in anacute care setting during past 5 year period.
- Experience in Oncology / Hematology (2 years minimum)
- Completion of the Desouza Chemotherapy / Biotherapy Course to becompleted, will be provided
- Excellent communication skills.
- Demonstrated commitment to the principles of Patient and FamilyCentred Care.
- Current Basic Cardiac Life Support (BLS) Certification.
- Regular and sustained attendance.
- Knowledge of and adherence to the Hospital's Code of Conduct.
- Computer skills using Word, Excel and Power Point, Groupwise.
Salary Range : $65.210 to 70.750
