Overview

The Personal Support Worker (Home Care) is a valued member of the home care team who works in partnership with a Care Designer and the broader care team to provide personal care and daily living support that helps clients age safely, comfortably, and happily in their own homes.
The Personal Support Worker is responsible for observing clients, communicating changes, and accurately documenting care provided. Assignments are designed to promote quality, continuity, and safety of care while respecting each client’s preferences, routines, and personal goals for aging well at home.
Essential Duties And Responsibilities
The Personal Support Worker delivers care with professionalism, accountability, and Bold Kindness, including but not limited to :
- Provides client care in alignment with the established plan of care, with clinical guidance and support from the Director of Nursing or registered nurse as required.
- Supports personal care needs in a respectful, person-centered manner, including :
- Bathing and personal hygiene
- Grooming, including oral care
- Assistance with dressing and undressing
- Assistance with toileting and continence care
- Assistance with feeding and meal support, including feeding clients who are unable to feed themselves
- Takes and records vital signs (temperature, pulse, respirations, and blood pressure) as required by the plan of care.
- Measures and records height and weight when necessary.
- Supports skin integrity and mobility by assisting with repositioning, encouraging movement, and following skin care protocols.
- Assists clients with active and passive range-of-motion exercises as outlined in the plan of care.
- Supports safe ambulation, transfers, and use of mobility aids.
- Observes and communicates changes in the client’s physical, mental, or emotional condition to the appropriate clinical lead in a timely manner.
- Documents care provided and observed changes accurately and promptly.
- Measures and documents intake and output as required.
- Provides medication reminders to support client self-administration, as permitted by scope.
- Performs light housekeeping to maintain a clean, safe, and comfortable home environment.
- Assists with meal preparation and clean-up in alignment with client preferences and care plans.
- Changes bed linens and helps maintain an orderly and welcoming client environment.
- Follows principles of infection prevention, control, and universal precautions at all times.
- Participates in performance improvement activities, recognizing that high-quality care is built through learning and consistency.
- Provides transportation support to medical appointments, community outings, and shopping as required.
- Participates in client education according to the plan of care, reinforcing safe practices and independence.
- Assists in maintaining a safe environment and promotes quality care through adherence to established policies, procedures, and standards, working collaboratively with the care team.
- Upholds client rights at all times, including the right to privacy, respect, choice, and confidentiality.
- Responds appropriately and promptly in emergency situations according to training and policy.
- Uses equipment and supplies safely and appropriately.
Qualifications
Certification as a Personal Support Worker or Home Care Aide from a recognized community college or vocational school.Minimum of one (1) year of relevant experience preferred.Current CPR certification.Negative TB skin test or chest x-ray, as required.Valid driver’s license and access to an insured vehicle in accordance with organizational requirements.Ability to work independently with minimal direct supervision while remaining connected to the care team.Demonstrated empathy, reliability, and respect for the needs of clients and families.Effective oral and written communication skills in English.Commitment to treating clients, team members, and the public with courtesy, respect, and professionalism.Ability to work collaboratively as part of a team.Commitment to maintaining confidentiality and security of client information.Working Conditions & Physical Demands
Regular contact with clients in a variety of settings, primarily within the client’s home and occasionally in institutional environments such as hospitals.May be required to respond to emergency situations.Travel is required.Position includes potential exposure to blood and other potentially infectious materials (OPIM).Personal Protective Equipment
May include gloves, masks, and eye or face protection as required to safely perform care and follow infection prevention and control practices.Physical Requirements
