Job Description

The Central West Ontario Health Team (CW OHT) is a collaboration of health partners across Brampton, North Etobicoke, West Woodbridge, Malton and Bramalea. As one of the largest OHTs in the province, it serves close to 900,000 people. Together, over 275 partners including home and community care, acute care, specialty care, primary care, long-term care, mental health and addictions services, patients/clients and community support services, have come together with the goal of providing better coordinated care for the community.

The Palliative Care Clinical Coach will be focused on improving access to palliative care for marginalized and underserved communities in the Central West region, across acute and community care settings. This role will work closely with CW OHT partners, including Osler’s acute care palliative team. Overall, the Clinical Coach is responsible for working with participating community organizations in alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. The Clinical Coach will drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the community (The Delivery Framework) and the Ontario Palliative Care Competency Framework.

Accountabilities:

  • Ensure priority organizations across long‑term care, community and hospital sectors have access to education, training and resources, while coordinating and delivering clinical education and coaching aligned with the Adult Model of Care and Ontario Health directives
  • Build competency amongst front-line providers from priority organizations in the delivery of primary-level palliative care, with a focus on:
    • Early identification of palliative care needs
    • Goals of Care conversations
    • Provide palliative care mentoring for health service providers in the long-term care, community and hospital sector settings in alignment with the Ontario Palliative Care Competency Framework
  • Work alongside the CW OHT Palliative Clinical Lead and Project Manager to support change management and quality improvement activities across priority organizations in the long-term care, community and hospital sector settings, in alignment with Ontario Health and CW OHT’s priorities
  • Participate in data collection and reporting when required, in alignment with provincial expectations
  • Provide support to Indigenous communities and organizations for their implementation efforts, as requested by those organizations and, where appropriate, in collaboration with regional partners


Qualifications

Qualifications:

  • Undergraduate degree in a relevant health or related discipline
  • Current registration as a regulated health professional (e.g., RN, RPN, NP, RSW) in good standing with the applicable regulatory college
  • Minimum five years of experience in palliative care practice
  • Previous experience supervising clinical staff and working across diverse health care settings and disciplines
  • Demonstrated experience in clinical coaching, consultation, mentoring, debriefing and supporting practice change
  • Experience designing, delivering and evaluating education and training for interprofessional learners, using evidence-informed and culturally responsive approaches
  • Experience with quality improvement initiatives, research and/or program evaluation, including the application of evidence-based practices
  • Strong interpersonal, communication, coaching and relationship-building skills, with demonstrated ability to work independently and collaboratively
  • Strong critical thinking, clinical judgement and problem-solving skills
  • Knowledge of relevant legislation, regulations and reporting requirements related to professional practice, including the Fixing Long-Term Care Homes Act, Excellent Care for All Act and Compassionate Care Act
  • Proficiency with Microsoft Office and educational technologies
  • Knowledge of the Ontario Palliative Care Competency Framework and/or experience applying palliative care standards and frameworks is an asset
  • Additional training in palliative care, gerontology and/or project management is an asset
  • Access to reliable transportation, as travel to priority organizations within the assigned geographic catchment area is required
