Job description

Who we are:

Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.

During that time it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges from information and education to case management psychosocial rehabilitation housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that in addition to dealing with client mental health issues address social determinants of health using the recovery model.


Overview: This position will assist with the oversight and management of the day-to-day
operations within Case Management and Community Services: Case Management triage Intake Hospital to Home and Property.


Requirements:
Participates as a member of a high performance multi-service team that supports
mental health clients within a variety of community settings
Responsible for scheduling the day-to-day activities within Case Management and
Community Services
Assists with ensuring activities are planned delivered and evaluated in accordance with
professional standards good operating principles best practices and legislative
requirements
Assists with preparing work plans that align with the organizations goals and objectives
Assists with directing staff providing support and guidance to facilitate: clinical
assessments system navigation referral relationship management with Lakeridge
Health Ontario Works Landlords Superintendents and Paralegal services collating of
workorders and invoices for approval and processing etc.
Assists with coordinating the client admission treatment and discharge process
Mentors staff and assists the Lead with supervision and leadership of staff
Participates in interview teams hiring and orientation of new employees
Assists with identifying the training and development needs of staff as required
Assists with ensuring all accreditation and accountability requirements are met
Assists with overseeing staff coverage to meet client care and unit needs
Assists with auditing ensure accuracy and adherence to CMHA Durham policy and
procedure
Facilitates individual and group trainings as required
Creates and maintains a positive work environment where all employees can maximize
their job satisfaction and productivity
Assists with the development and implementation of Case Management and Community
Services policies and procedures
Facilitates team meetings focusing on business and operational issues as required
Provides on-call crisis intervention during night/weekend hours and acts as back up to
evening and weekend staff as required
Support of and participation in the Annual Unit Inspection process
Support of and participation in any Landlord Tennant Board involvement
Supports and monitors the Property HelpDesk system
Performs other tasks as assigned by management


RISK MANAGEMENT
Minimizes risks adhering to CMHA Durham Policies and Procedures identifying assessing
evaluating and controlling risks ensuring well-being of staff guests caregivers and clients


Skills:
Post-secondary education in a related area of study (i.e. mental health social sciences
human services property management) and a minimum of five years related experience
or equivalent combination of education and experience
Sound knowledge of the Mental Health Act Landlord and Tenant Act Health Care
Consent Act social service benefits and related legislation in Ontario
Understanding of the Landlord and Tenant Board and N forms
Understanding of Property Management functions (basic unit maintenance and
procurement)
Ability to provide staff supervision and leadership
Excellent verbal and written communication skills with all levels within and outside the
organization.
Able to develop and maintain positive relationships with clients community partners and
represent the agency in a professional manner at all times
Ability to acquire and utilize community resources effectively to meet clients needs
Flexibility to adapt to change
Experience working with diverse communities and backgrounds.
Ability to work independently and in a dynamic team environment.
Understanding of CMHA Durham HelpDesk and Ticket System
Knowledge of psychotropic medications and their effects
High level of critical thinking and logical thinking.
Excellent organization time management and prioritizing skills.
Ability to manage crisis situations.
Valid Ontario Drivers license access to a dependable vehicle in safe working order and
appropriate insurance coverage
Knowledge of MS Office (Word Excel PowerPoint Access and Outlook) as well as the
internet


Working Conditions:
Works on site in office settings and community settings
Exposure to abusive/irate clients distractions interruptions and deadlines
Little exposure to hazards
