Job description
Transform community health as a proactive Family Physician. Your role will emphasize accessibility, health promotion, and addressing complex patient needs in a supportive environment. This position involves delivering comprehensive medical care within an interprofessional team, enhancing the well-being of underserved populations. Your commitment to understanding the social determinants of health will be crucial in creating meaningful connections with clients and improving their health outcomes. Key Responsibilities: • Provide holistic primary care to a diverse clientele • Conduct assessments and develop treatment plans • Collaborate with healthcare professionals for integrated care • Participate in community outreach and health education • Maintain thorough medical documentation Requirements: • Medical Degree with Family Medicine training • Active CCFP certification and good standing with CPSO • Minimum two years working in a community health setting • Experience with marginalized communities • Current Ontario Driver's license for flexibility in care Make a significant impact in primary care as a Family Physician, advocating for health equity while providing exceptional care tailored to community needs. #J-18808-Ljbffr
