Role : PSW Team Lead

Full Time, permanent position

Monday Friday 6am 3pm + weekend rotation (1-2 weekdays off after weekend rotation based on business needs)

Location : Retirement Home located in Stoney Creek

Responsibilities :

  • Performs personal care and approved delegated tasks to promote the physical, social and emotional well being of each client.
  • Team lead to provide initial contact with new admissions, reviewing current care plan to see if updates are needed and collaborating with MCP.
  • Observes and reports significant changes in the client’s physical and emotional condition promptly to MCP for the facility and to facility nurse as appropriate
  • Cover book off, vacations of fellow PSW’s and notify facility MCP and CSC via email of no shows or schedule changes.
  • Reorganize client visit times and provide direction to PSW’s in the event of staff shortage.
  • Participates in Team Meetings held by the facility MCP .
  • Attends LHIN meetings, in-service education sessions and Transfers of Authority, etc. as directed by the Manager of Clinical Practice.
  • Provide mentorship for PSWs as needed.
  • Reports health and safety hazards to responsible MCP
  • Meet daily or as needed with facility staff to report any client condition updates.
  • Update Bayshore Binder with any changes.
  • Performs all delegated duties under the direction of the Manager of Clinical Practice
  • Performs other delegated duties as assigned.

Job Qualification

Who you are :

  • Strong commitment to improving community well-being.
  • Minimum 2 years' experience in home health care.
  • Supervisory experience preferred.
  • Accredited PSW Diploma or equivalent education.
  • Valid driver’s license and reliable vehicle.
  • Clear Vulnerable Sector Check or willingness to obtain.
