Detailed Overview

In accordance with Fraser Health's Vision, Mission and Values, program mandates and clinical practice guidelines, with an emphasis on recovery and rehabilitation and reducing barriers to re-enter the system of care, supports individuals and families affected by acute/episodic mental illness and substance use disorders. Focuses on advocacy, supportive counselling, psycho-education, community resourcing, and care coordination support within the local service area across the care continuum, to ensure delivery of consistent, relevant and timely treatment, rapid re-engagement, and access to information where appropriate and feasible. Contributes to program development and provides education within the assigned program. Services may be provided on an outreach basis when required to assess and/or engage clients within their own environment and community.

As a clinician in a shared-team model, provides intake, screening, and assessment services, treatment recommendations and plans, determines referral needs, provides follow-up support, group education, liaison with community resources for specialized services, counsels clients, provides outreach as required, and is assigned as the most responsible clinician to specific clients dependent on the client's primary needs. Provides client- and family-centered care through a trauma-informed approach and evidence-informed practice. Encourages knowledge exchange in day-to-day activities, empowers clients and natural supports to engage and connect within their communities, and ensures they are supported in navigating the health care system.

Responsibilities

  • Develops and implements recovery-oriented treatment options and plans in collaboration with clients and natural supports; provides comprehensive case consultation and follow-up support on treatment decisions in collaboration with other interdisciplinary team members; supports the development/maintenance of abilities related to activities of daily living, discharge and transition of care. Updates the Clinical Coordinator on client treatment status and progress, and on significant issues and developments.
  • Provides individual, group, and family support using interviewing, supportive counseling, and assessment methods. Shares responsibility among the team for workload management, crisis intervention, home visits, group facilitation, and outreach as required.
  • Supports the intake process and assesses client needs by gathering information/history, performing initial assessments, determining priority of care, eligibility, and required level of care; collaborates with the interdisciplinary team to determine best approach to care and assesses the need for emergent crisis intervention.
  • Facilitates client utilization of appropriate community services and rehabilitative programs, including housing support, referrals to community treatment options, and liaison with program providers; develops supportive and rehabilitative programs for adults and their families.
  • Advocates on behalf of clients by providing information about processes and completing forms; acts as a resource for clients and families.
  • Coordinates hospital admissions when required, including involuntary admissions where appropriate; arranges transportation and liaises with health professionals and authorities; ensures necessary forms and communications are completed.
  • Provides linkages between community mental health & substance use services and acute care facilities to facilitate discharge planning and continuity of care; collaborates with forensic programs as needed.
  • Delivers individual and group sessions, including psycho-education or skill development; involves families in information sessions and provides staff, clients, and families with education about illness impact and recovery.
  • Maintains client records in clinical information systems, prepares progress notes and reports, and ensures privacy and regulatory compliance.
  • Participates in quality improvement, risk management, and ongoing education to maintain current practice and knowledge of applicable legislation.
  • Participates in local committees/advisory boards and information-sharing activities; contributes to program development and representation of the team.
  • Participates in clinical research and program evaluation activities as requested.
  • Performs other related duties as assigned.

Qualifications

Education and Experience

Bachelor's degree in Social Work, Clinical Counselling or other relevant health-related discipline. Two (2) years' recent, related experience working with clients and families affected by mental illness and substance use disorders, or an equivalent combination of education, training, and experience. Eligible for registration or membership with applicable professional association or regulatory body. Valid Class V BC Driver's License and access to a personal vehicle for business-related purposes.

Skills And Abilities

  • Demonstrated client assessment and case management skills.
  • Effective verbal and written communication, with empathic and respectful approach.
  • Strong interpersonal skills and ability to collaborate with clients, families, and interdisciplinary teams.
  • Ability to manage a heavy caseload with sound clinical and administrative judgment.
  • Adaptability to changing caseload needs and working under pressure.
  • Ability to work independently with appropriate supervision when needed.
  • Commitment to confidentiality and privacy in accordance with policies.
  • Ability to learn new methodologies quickly and apply them.
  • Knowledge of community resources and methods to access them.
  • Knowledge of emergency triage, mental health assessment, and counseling principles.
  • Working knowledge of psychopharmacology, crisis theory, and de-escalation techniques.
  • Working knowledge of DSM-5/ICD classifications.
  • Physical ability to perform duties as required.
