PURPOSE>

We are currently recruiting for a Social Worker to provide clinical counselling and case management as a part of our Bloom team. Bloom Clinic is one of several provincially funded Hepatitis C outreach teams providing testing, treatment, and care to people who use drugs and people who are street involved across Peel Region. The successful candidate will have a strong foundation in harm reduction and work from an anti-racist, anti-oppressive, and trauma informed framework.

RESPONSIBILITIES

• Provide counselling and case management to individuals living with, at risk of, and affected by HIV and HCV across Peel Region
• Support clients to develop goals and provide appropriate client-centred interventions
• Provide community‑based education sessions on various issues including HIV, HCV, Harm Reduction, Mental Health, and more.
• Screen / assess for self‑harm / risk factors and create safety plans with clients as needed.
• Engage in individual and systemic advocacy related to needs articulated by community members
• Determine need for additional supports and make appropriate external referrals as needed.
• Build and maintain strong relationships with community partners.
• Maintain accurate and up to date client records in the EMR.
• Ensure comprehensive treatment through ongoing consultation and collaboration with team members.
• Actively participate internal meetings as well as external community tables and committees
