Shift times : Rotating 08 : 18 to 16 : 30.

About The Role
The Community Integration Care Coordinator (CICC) practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses & Midwives (BCCNM) as well as within a client and family centered care model.
The CICC is responsible for the coordination of day to day operations and quality client care within designated clinical areas and supports the seamless movement of clients through the healthcare system and in accessing appropriate resources. The CICC supervises as well as serves as a role model and a resource to staff for clinical decision-making, the clinical assessments of clients and in the counselling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of client care, adequate staffing levels, and a continuous quality improvement environment. Liaises with an interdisciplinary team for the identification, coordination and efficient utilization of resources.
