Hours Worked : Monday-Friday : 8 : 30am-4 : 30pm

Responsibilities

• The Case Worker (Activity / Programming) is responsible for assisting in developing, coordinating, and facilitating programming and events that are informed by participant recommendations and identified needs. Working primarily in the Drop-in, the Case Worker is responsible for providing direct support to individuals accessing the space, contributing to the service plan process, and collaborating with Drop-in Support Mentors and Case Management Teams to contribute to the clear understanding of support needs and associated program goals.
• As a part of a team, Case Workers are expected to uphold agency values and provide respectful and holistic support while utilizing a strength-based and conflict prevention approach. Other duties include maintaining positive relationships with participants, identifying programming needs to the Program Coordinator, providing leadership and guidance, identifying internal and external resources, communicating effectively with program teams, and creating programming that considers a spectrum of needs, capabilities, and interests.
• With Clinical Case Managers, Case Workers may prepare, maintain, and provide required internal reports, files, and records, submitting such reports as required, including overseeing individual participants’ record keeping. The Case Worker also builds connections and works collaboratively with other programs, staff, external agencies, community groups, and community resources.
