About the Role
Reporting to the Sr. Director, Implementation & Operations, the Community Health Navigator (CHW) will act as a trusted resource for health education, outreach, and navigation of healthcare and social services within McKesson Canada’s Trenton Distribution Centre and support INVIVA programs.
Location & Schedule
- City : Trenton, Ontario (Trenton Distribution Center) – 211 North Murray Street
- City : Mississauga, ON (INVIVA Program) – 2300 Meadowvale Blvd
- Hours / Week : 25 hours on-site (Trenton) plus 15 hours hybrid / virtual (INVIVA)
- Language : French and English fluency required.
Key Responsibilities
Provide personalized health guidance and culturally sensitive support to Distribution Center associates.Promote health literacy and engagement strategies.Maintain accurate documentation and data integrity.Help associates navigate healthcare systems, social services, and community resources.Build relationships with community organizations and participate in outreach.Support special projects and additional responsibilities as needed.Coordinate with INVIVA RNs, Program Leadership, and vendor partners to manage patient referrals, enrollment / unenrollment, and reporting.Ensure secure handling of patient referrals per protocol and support selection committee activities.Assist with reports for INVIVA RNs or Program Leadership as needed.Attend monthly meetings with Program Leaders to review progress.Maintain confidentiality, privacy, and ethical standards throughout.Working Conditions
On‑site, in‑person work within a traditional distribution center environment; primarily computer-based with occasional warehouse walkthroughs.Occasional travel for community program collaborations, outreach, and INVIVA related activities.Flexible availability for multiple shifts and occasional travel.Qualifications & Experience
Minimum 3 years of relevant experience; High School Diploma required. CHW certification or training preferred.Proficiency in Microsoft Office Suite.Strong interpersonal and communication skills.Experience in community outreach and health advocacy.Ability to guide and support individuals in accessing health resources.Problem‑solving and decision‑making skills.Highly organized and proactive.Commitment to privacy, confidentiality, and ethical standards.Professional communication across all organizational levels.Adherence to company policies and accurate metric tracking.Demonstrate behaviors aligned with McKesson Canada’s I²CARE & ILead principles.Compensation
Base pay range : $64,200 – $106,900.
