Community Health Navigator

About the Role

Reporting to the Sr. Director, Implementation & Operations, the Community Health Navigator (CHW) will act as a trusted resource for health education, outreach, and navigation of healthcare and social services within McKesson Canada’s Trenton Distribution Centre and support INVIVA programs.

Location & Schedule

• City : Trenton, Ontario (Trenton Distribution Center) – 211 North Murray Street
• City : Mississauga, ON (INVIVA Program) – 2300 Meadowvale Blvd
• Hours / Week : 25 hours on-site (Trenton) plus 15 hours hybrid / virtual (INVIVA)
• Language : French and English fluency required.

Base pay range : $64,200 – $106,900.
