Job Description

The Patient Care Coordinator for Ambulatory is a member of the program / clinical area management team who float across Mackenzie Richmond Hill Hospital and Ambulatory clinics.

Patient care coordinator is responsible for the overall day-to-day program-based coordination of patient and family care, facilitation of patient flow in and out of the ambulatory program, coordination of clinical staff and resources, acting as a clinical resource for the physicians, nurses and interprofessional team as well as supervisory support functions such as attendance and performance management and conflict resolution at the point of care and beyond.

Patient care coordinator will coordinate and assist physicians in primary care and in Long Term Care Homes in navigating services, providing centralized access to hospital and community services, with follow-up on status of referrals, results, and consults.

This role supports timely access, ensures navigator to appropriate resources, and reduced ED visits as patients are proactively connected to the right level of care

What must you have?

  • Registered Nurse with Bachelor of Science of Nursing (BScN) Degree or currently in the process of completing the BScN.
  • Current general certificate of registration and member in good standing with the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certificate required.

What else do you bring?

  • Minimum of two years of relevant acute inpatient experience
  • Experience with QBP pathways and management of this patient population.
  • Experience with case management, and discharge planning
  • Demonstrated evidence of understand and application of principles of quality care and outcome measurement.
  • Able to discern relevant information to make effective decisions regarding patient care and program planning.
  • Knowledge of CNO standards of practice and an understanding of the application of the RHPA legislation.
  • Ability to work independently / be self-directed and effectively lead clinical inter-professional teams.
  • Highly developed conflict resolution skills.
  • Well-developed ability to delegate tasks appropriately.
  • Demonstrates commitment to professional development and learning.
  • Demonstrates excellence in interpersonal, communication, coaching, mentoring and team building skills that facilitate positive interactions among all members of the inter-professional team.
  • Demonstrates understanding of continuous improvement principles and tools.

  • Ability to analyze research and implement evidence-based practice into Policy and Procedure development and clinical practice.
  • Experience with guiding staff experiencing performance and / or attendance problems.
  • Communicates effectively using timely and appropriate written, verbal and non-verbal means.
  • Utilizes effective listening skills to maximize understanding of issues.
  • Proven ability to work within a computerized environment.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job.
  • Effectively uses empathy in interactions with others.
