Demonstrates good judgment, problem-solving, and decision-making skills. Proficient in computer skills, Microsoft Office, and Sunrise applications.
Job Description
The Wellness Nurse at Sunrise Senior Living is responsible for promoting the overall well-being of our residents. We are looking for a dedicated and compassionate individual to join our team.
Responsibilities :
- Identify ongoing needs and services of residents to deliver high-quality resident care.
- Ensure proper follow-through and evaluation of residents with changes in healthcare services and status.
- Assist the Resident Care Director in conducting resident evaluations and gathering data.
- Conduct monthly wellness visits for all residents.
- Monitor and document incidents, falls, or accidents, reporting as required.
- Maintain effective communication with residents' families and responsible parties.
- Contact attending physicians when necessary.
- Ensure weights and vital signs are obtained monthly for each resident.
- Assist in maintaining wellness files according to Sunrise policies and regulations.
- Provide clinical support and assistance to community team members as needed.
