Job Title : Nurse Practitioner

Position Overview : The Nurse Practitioner (NP) will play a key role in the primary care team, providing expert health services to older adults (65+) with complex health conditions. Reporting to the Program Manager, the NP will work independently and collaboratively within the multidisciplinary team of the Elderly Health and Transition Services (EHTS). The role involves providing community-based outreach, health promotion, disease prevention, and health education to support older adults in managing their physical and mental well-being.
Key Responsibilities :
Conduct comprehensive health assessments, including a holistic health history, physical examination, and diagnostic testing, for older adults across the lifespan. Develop individualized treatment plans addressing both physical and mental health needs within the scope of services provided.
Assess and manage common chronic conditions in primary healthcare, offering both episodic and long-term care. Collaborate with Community Health Centre (CHC) physicians for complex cases beyond the Nurse Practitioner scope.
Provide telephone consultations for intake, referrals, and healthcare advice for both acute and chronic conditions. Offer anticipatory guidance and support for clients, ensuring timely and accurate communication.
Deliver preventive care counseling, brief supportive counseling, and short-term crisis intervention as necessary. Provide lifestyle and health education, including on nutrition, mental health, and wellness.
Write prescriptions, order laboratory tests, and perform diagnostic evaluations within the Nurse Practitioner’s authorized scope of practice.
Monitor and administer medications, conduct medication audits, and contribute to the development of organizational medication policies.
Conduct home visits to clients within the service area and provide referrals to appropriate specialists, hospitals, or community services.
Actively participate in community projects, agency activities, and case conferences with other healthcare providers to coordinate comprehensive client care.
Contribute to the development, planning, and evaluation of clinical programs and policies. Support the assessment and review of services.
Provide clinical care in outreach settings as needed and act as a backup to other nursing staff when required. Engage in infection control practices, clinical equipment maintenance, and sterilization procedures.
Maintain thorough, accurate, and timely client records. Contribute to data collection and statistical reporting as needed.
Participate in the development of new clinical policies, attend special events, and carry out other duties as assigned.
Required Qualifications and Competencies :
A degree in nursing from a recognized university or equivalent education and experience. Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario (CNO) and a certificate of continuing competence.
A minimum of 3-5 years of experience in community healthcare, or a combination of relevant hospital or public health nursing experience.
Workplace Conditions :
Typically, a 35-hour workweek, with some flexibility required depending on client needs. Evening, weekend, and holiday availability may be necessary to accommodate clients.
Exposure to infectious diseases, including AIDS and hepatitis B, and other healthcare-related risks.
Attendance at special events may be required from time to time.
