Job descriptionHave a degree in Social Work or a Social Service Worker diploma. Maintain current registration with the Ontario College of Social Workers & Social Service Workers. Ensure effective safety procedures are developed and managed to comply with statutory obligations, in conjunction with relevant organizational policy. Coordinate and support the transition of residents moving into our Village, provide guidance in areas such as loss, life transitions and the Resident Bill of Rights. Support resident and family meetings, care conferences, therapeutic groups, and engage with outside agencies and community groups as needed. Document appropriate resident information and participate in the development of comprehensive plans of care. Provide mediation, education, training, coaching and counselling for residents, families and team members regarding mental health and well-being, while serving as a key representative for Resident and Family Council. Support residents on the waitlist and ongoing assessment needs with residents and those waiting for services. Current Heart Save Level of CPR Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety Act of Ontario. Demonstrated experience working with people living with cognitive or physical impairments. Experience working with a geriatric population.
