Schedule

OPCOM09: $ - $ per hour. Annual Salary: $93, - $122,772 per year.

Patient care

MSH provides services to a wide range of patients in the following areas: Surgery, Medicine, ICU/CCU, Childbirth and Children, Psychiatry, Rehabilitation, Palliative, Complex Care, Oncology and Emergency. The Uxbridge Hospital provides services in Medicine, Complex Care, Emergency and Day Surgery. Support

As a clinical pharmacist at Oak Valley Health, you will work with the inter-professional team to provide clinical pharmacy services to Markham Stouffville Hospital (MSH), Uxbridge Hospital and Reactivation Care Centre (RCC) patients. In addition, you will participate in patient education, support teaching students and provide drug information to nursing and medical staff. You will work collaboratively with the pharmacists and pharmacy technicians as an integral member of our pharmacy team.

Qualifications

Graduated from an accredited university pharmacy program. Licenced under Part A registration status with the Ontario College of Pharmacists. Hospital pharmacy residency is preferred. Previous experience in a hospital setting with unit-dose and IV admixture programs is required. Demonstrated experience identifying, resolving and preventing drug related problems to optimize medication therapy and safety. 48_Creative Created with Sketch.
