Accountabilities :
- Work collaboratively with the Pharmacy and Antimicrobial Stewardship Teams to provide patientcentred care
- Provide education to pharmacists physicians and other hospital staff
- Provide drug information to physicians nurses and other clinicians to enhance patient care
- Provide education to patients to promote optimal drug use
- Mentor Pharmacy residents students and interns
- Participate on hospital committees as required
- Resolve daytoday clinical issues
- Assist with the implementation of pharmacy and antimicrobial stewardship initiatives which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations
- Contribute to the development of new program initiatives and interventions
- Perform daily prospective audit and feedback of patients receiving antimicrobial therapy
- Collect interpret and analyze relevant metrics and indices related to antimicrobial usage and microbiologic trends
- Other duties as assigned
Qualifications :
Currently licensed (or eligible) by the Ontario College of Pharmacists Part A of the registryCompletion of hospital pharmacy residency or equivalent experience is preferredAdditional training or experience in infectious disease or antimicrobial stewardship an assetAdvanced computer skills including Microsoft Excel and PowerPointExcellent attendance punctuality and work recordDemonstrated ability to work in a multidisciplinary teamMotivated selfdirected learnerStrong clinical assessment skills in identifying and resolving drugrelated problemsDemonstrated ability to apply clinical judgment in analyzing and solving clinical problemsMay be required to travel between and work at all Osler sitesExcellent oral interpersonal and written skillsMust be able to meet the physical demands of the positionMust demonstrate Oslers Values of Respect Excellence Service Compassion Innovation and CollaborationAdditional Information :
