Job description

Position: Registered Nurse
Department: Regional Dementia Support & Transitional Services {Kirkwood Place – Sudbury}
Duration Approximately Ten (10) Months

The Regional Dementia Care Program provides comprehensive specialized assessment and treatment of older adults (usually >65 years old) and/or adults with an age-related dementia, which is complicated by behavioural and psychological symptoms, and/or medical comorbidities that exceed the capacity of the community, general hospital, complex continuing care and long-term care system. Nickel Lodge is a Specialized Adult Rehabilitation and Transitional Service which provides care and support to those with a severe mental illness who often present with a co-morbid condition and a focus on concurrent disorders to substance use. It is a specialized unit that is patient driven, recovery oriented and integrates patient wishes and goals through a wellness framework.

QUALIFICATIONS

Certification/Experience/Knowledge:

  • Current Certificate of Registration ~ College of Nurses of Ontario in good standing required
  • Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve months)
  • Current Gentle Persuasive Approach (GPA) Certificate required
  • Experience working with older adults demonstrating responsive behaviours including evidence of creative application of Personhood required
  • Knowledge of neurodegenerative disorders, best practices, and relevant pharmacotherapy for this population required
  • Knowledge of Recovery Oriented Practice Supporting Patient Choice
  • Knowledge and experience supporting individuals with concurrent disorders required
  • Knowledge of relevant acts and regulations e.g. Regulated Health Professionals Act, Mental Health Act, PHIPA required
  • Understanding of trauma informed care principles and able to apply within all relationships and interactions
  • Motivational Interviewing, trauma informed care and Applied Suicide Intervention Skills Training (ASSIST) training an asset
  • Experience providing assessment, treatment, and rehabilitation for individuals with complex and persistent mental health problems
  • Knowledge of The Recovery Model used to optimize reintegration to the community
  • Knowledge of supporting individuals experiencing substance use disorders concurrent to a serious and persistent mental health concern; specific programming is available for these individuals
  • Educational assets: Gentle Persuasive Approach (GPA), PIECES, Montessori Methods for Dementia

Skills/Abilities:

  • Demonstrated application of the principles of psychosocial rehabilitation in an adult psychiatric population required
  • Excellent Verbal de-escalation skills required
  • Must be willing to work shifts in other programs as required
  • Must demonstrate excellent level of interpersonal skills
  • Must demonstrate excellent level of written and verbal communication skills
  • Ability to work both independently and collaboratively with all members of the multidisciplinary team, clients, families, regional support services and community partners required
  • Ability to provide health teaching to individuals and groups required
  • Must be committed to continuing education on an annual basis and demonstrate commitment to quality improvement required
  • Ability to effectively assume a leadership role as a collaborative member of the health team required
  • Ability to assume responsibility for patient care in a total patient care climate required
  • Able to meet the physical demands required to provide nursing care to all patients required
  • Ability to problem-solve and function in fast-paced environment required
  • Excellent demonstrated assessment, critical thinking and intervention skills required
  • Must demonstrate basic computer knowledge
  • Must demonstrate commitment to ensure personal safety, safety of fellow workers, patients, volunteers, and visitors
  • Ability to support and contribute to a culture of safety and prevention of adverse health events required
  • Must demonstrate good attendance and work record
  • Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
